At your first visit you will need to bring your completed patient forms as well as your insurance card and identification. It is important for you to bring this information with you each time your child has an appointment whether your child is seeing one of the physicians, a nurse practitioner, a physician assistant or one of the audiologist.
PENTA participates with most HMO and PPO insurance companies and as a convenience to you, we will verify your insurance using the information you provided when you scheduled your child’s first appointment. If we are unable to verify your insurance, you will be responsible for payment in full at the time of the visit. Co-payments, co-insurance, and deductibles are also due at the time of visit.
Also, please refer to your insurance handbook for specific benefits (what your insurance covers and pays) and limitations (what your insurance does not cover or pay) of your health insurance policy.

The following insurance plans require a referral. Please be sure to have your referral at the time of your visit.

- KAISER – HMO
- UHC COMPASS/NAVIGATE
Unfortunately, PENTA does not accept the following plans:
- BCBS PATHWAY – NOT ON PLAN
- HUMANA HMO X, EPOX, PPOX
